We are a group of independent clinicians who provide assessment, treatment, and holistic health services and consultation to individuals, children, youth, couples and families. Each provider works as an independent clinician and is responsible for his or her own private practice, fees and hours. Each clinician brings his or her own unique experience based on individual training and education. All clinicians and practitioners offer a free initial phone consultation.

SELF-PAY RATES:
$135.00 - 1 hour session;
$175.00 - 90 minute session;
$100.00 - reduced rates for hardship.
INSURANCE: Dr. Hayes takes United Healthcare, OPTUM, Blue Cross Blue Shield, Aetna and limited EAP.

$75.00 - $85.00 1 hour for hardship.
INSURANCE: Medicare, United, Optum, Blue Cross/Blue Shield and most EAP.

Our facility is open by appointment only most days, with evening and weekend appointments available-depending on each clinician. Each independent clinician sets his or her own schedule but the above is generally when we are open.
To make an initial appointment please call Sandy at 520.981.4547. Please note that Sandy answers calls and emails Monday through Thursdays by 4:00.
